Description of the information about Memorial/Dedication/Rededication Services for the Confederate Veteran is enter used the following section of the form. Note that the Confederate Graves Registry supports multiple Memorial Service entires.

SCV/UDC: SCV/UDC, Camp/Chapter Number, City and State. Information for sponsoring SCV Camp or UDC Chapter for the Service. Abbreviations for State should be used, for example GA to improve search results. If the sponsoring organization is the SCV or USC these should be left blank and the Other Oranization information entered instead. All fields can be used as search terms.

Other Organization: Name, City and State. Name of non SCV/UDC organization, if SCV/UDC use above and leave this section blank. Abbreviations for State should be used, for example GA to improve search results. All fields can be used as search terms.

Date of Service: Year, Month and Day. "unknown" is supported in each field so partial information can be entered. Year should be the four digit number such as "2001." Service Year can be used as a search term.

Add Memorial Service/Dedication/Re-dedication: Use this check box to Add another Memorial Service record for this veteran. When this box is checked and the veteran form is submitted or saved, it will be redisplayed with an additional Memorial Service entry. There are no practical limits on how many Memorial Service entries can be associated with a Confederate Veteran.
